At the Princeton Biofeedback Center, appointments are scheduled on the
hour. If you need to cancel your appointment, kindly give the office 24
hours notice.
Please arrive 15 minutes prior to your initial appointment. Please bring
your INTAKE CONSULTATION FORM and your INSURANCE CARD
with you for your first appointment along with any other paperwork you
feel necessary for the doctor to see.
*The INTAKE CONSULTATION FORM MUST be filled
out in it's entirety for your initial appointment.

At the present time we accept Aetna Insurance. Please be sure to call your insurance
carrier to confirm that Aetna manages your mental heath benefits.
We also participate in the Shared Savings/Multi-Plan, available with certain insurance
carriers. To check if your insurance carrier is enrolled with this plan, look on the backside
of your insurance card. If you see the SHARED SAVINGS/MULTIPLAN symbol or the
printed words then you may be eligible for the Multi-Plan discount. Please be sure to
mention this to the receptionist.
