At the Princeton Biofeedback Center, appointments are scheduled on the hour.  If you need to cancel your appointment, kindly give the office 24 hours notice.

For your initial appointment at the Centre, a completed Intake Consultation Form is required.  It is very important that this form be filled out PRIOR to your scheduled appointment. 

If you cannot print and fill this form out before your appointment, please arrive to our office 30 minutes before hand to give yourself time to fill all paperwork out.

If you are unclear or confused with any part(s), leave them blank and Dr. Fehmi will work through the form with you during your Intake session.
